As with any physical activity, there is risk of injury. I fully accept this risk and hold harmless from any legal liability, First Baptist Church (FBC) of Canton and any persons involved in the VBS ministry. 2) In the event of an emergency that requires medical treatment for the above named child(ren), I understand every effort will be made to contact me or my emergency contact. However, if we cannot be reached I give permission to FBC volunteers to secure the services of a licensed physician to provide the necessary care for my child(ren)'s well-being. I assume responsibilities for all costs connected to any treatment of my child(ren). 3) I grant permission to First Baptist Church for the unrestricted usage of any likeness (photo or video) taken of the participant(s). Such use includes the display, distribution, publication, transmission or otherwise use of photos, images and/or video taken of the participant, not limited to printed material, internet usage, such as FBC website, YouTube, Facebook, etc. By signing (typing) my name below, I have read, understood, and agree to the Terms and Conditions stated above and give my permission for my child(ren) to attend VBS at First Baptist Church of Canton.
